Weight loss is about much more than the number on the scale. While reducing body fat may be the primary goal, maintaining muscle mass, strength and bone health is just as important – especially when weight is lost rapidly.

This has become particularly relevant with the increasing use of GLP-1 and dual GIP/GLP-1 medications such as semaglutide and tirzepatide. Effective pharmacological weight loss can lead to substantial changes within a relatively short period of time, making adequate nutrition and regular loading of the muscles and bones increasingly important.

Weight loss, weight-loss medications and muscle mass

As body weight decreases, some muscle mass may be lost alongside body fat. Preserving muscle during weight loss is important for maintaining strength, physical function and a healthy metabolism.

In the DXA substudy of the SURMOUNT-1 trial, body weight decreased by an average of 21.3% over 72 weeks, fat mass by 33.9% and lean mass by 10.9%. Approximately 75% of the weight lost was fat mass and 25% was lean mass.

When medication reduces appetite, total energy and protein intake may also decrease. During substantial weight loss, it is therefore important to ensure adequate nutrition and provide the muscles with a regular training stimulus.

Whole-body vibration (WBV) has also been studied in people with overweight and obesity. A systematic review and meta-analysis including 23 studies and 884 participants found that WBV training improved particularly lower-limb muscle strength, with additional favourable changes observed in outcomes including fat mass and blood pressure.

Another systematic review and meta-analysis found beneficial effects of WBV training on fat mass, particularly when it was combined with other weight-management strategies such as exercise and dietary interventions. A more recent systematic review found that combining WBV with other forms of exercise was associated with reductions in fat mass as well as blood triglyceride and cholesterol levels, alongside changes in blood pressure, heart rate and arterial stiffness.

Weight Loss and Bone Health

Substantial weight loss can also be accompanied by changes in bone health. As body weight decreases, the everyday mechanical load placed on the skeleton is reduced. Changes in energy and nutrient intake as well as muscle mass may also influence bone health.

Studies investigating weight loss and bone mineral density have found reductions in bone density, particularly at the hip. A systematic review and meta-analysis found that adding exercise to diet-induced weight loss attenuated the loss of bone mineral density at the femoral neck compared with diet-induced weight loss alone.

The relationship between GLP-1-based weight-loss medications and changes in bone health is being actively investigated, and current evidence does not yet provide a completely conclusive picture of their long-term effects on bone mineral density or osteoporosis risk. However, substantial and rapid weight loss is known to potentially be accompanied by reductions in bone mass and bone mineral density, making the possible association with osteopenia and osteoporosis risk important to consider during medication-assisted weight loss as well.

Supporting bone health becomes particularly important when weight is lost rapidly or when osteopenia, osteoporosis or other risk factors for bone fragility are already present. Muscle activity and exercise provide important mechanical loading for the skeleton, while Power Plate training can support muscle strength, balance and physical function.

Weight Loss and Body Toning

After significant or rapid weight loss, the skin may feel and appear looser than before. How well the skin regains firmness after weight loss depends on several factors, including age, genetics, the amount and speed of weight loss, nutrition and the condition of the skin.

Studies examining skin after massive weight loss have found changes in collagen and elastin fibres as well as the structural organisation of the skin. This may help explain why the skin recovers differently from one person to another.

Body tone is influenced not only by the skin but also by the muscle tissue underneath it. Maintaining and developing muscle mass helps support the body’s shape as body fat decreases.
